A 23-year-old female visited our department due to recurrent attack of headache and hemiplegia. The patient had experienced her first hemiplegic migraine-like attack at the age 18 years. The headache was accompanied by weakness
and tingling sensation in the right face and extremity without visual aura. These symptoms lasted for one hour and occurred about 10 times a year. Magnetic resonance imaging revealed a typical feature of cavernous angioma with a maximum diameter of 1.1cm in the left parietal cortex and electroencephalogram was normal. The cavernous angioma was totally removed through navigation-guided craniotomy. After removal of cavernous angioma, the patient did not experience hemiplegic migraine-like attack. Our case demonstrates a cavernous angioma presenting hemiplegic igraine-
like attack. Even though symptomatic hemiplegic migraine is rare, it is important to consider a secondary cause.
